ABSTRACT

Objective To evaluate the role of combination of three-vessel views in ultrasound diagnosis of fetal aortic arch and pulmonary arterial branch anomalies.Methods The data of 66 cases of fetal aortic arch and pulmonary arterial branch anomalies were retrospectively analysed,including echocardiographic data,autopsy and operation records and postnatal follow-up results.Echocardiogaphic features and display frequencies on three vessels and trachea view(3VT),three vessels view(3VV)and three vessels and pulmonary arterial branches view(3VP)were summarized.Results There were 52 cases of aortic arch abnormalities,including 32 cases of right aortic arch with left ductus arteriosus,4 cases of double aortic arch,7 cases of aberrant right subclavian artery,6 cases of coarctation of aorta and 3 cases of interruption of aortic arch.Fourteen cases were diagnosed pulmonary aterial branch abnormalities,including 1 0 cases of crossed pulmonary arteries,2 cases of anomalous origin of left pulmonary artery from aortic arch and 2 cases of pulmonary artery sling.The display frequencies of fetal aortic arch and pulmonary arterial branch anomalies on 3VT,3VV and 3VP were 80.3%,19.7% and 39.4%.Conclusions The combination of three-vessel views is of great value in prenatal diagnosis of fetal aortic arch and pulmonary arterial branch anomalies.

ABSTRACT

Objective To investigate the clinical value of prenatal ultrasonography in diagnosing fetal aortic atresia.Methods Ultrasound images of 9 cases with fetal aortic atresia confirmed by autopsy were retrospectively analysed and summarized.Results Fetal echocardiography showed ascending aorta and aortic arch were thin in seven cases and unclear in two cases.Color Doppler flow imaging showed fetal aorta no blood connection with left ventricle and reverse blood flow in aortic arch among all 9 cases.There were 6 cases associated with hypoplastic left heart syndrome,two cases associated with ventricular septal defect and well-developed left ventricle,one case complicated with complete atrioventricular septal defect.Nine cases were detected at average gestational age (26.0 ± 2.6) weeks,minimum gestational age was only 16 weeks pregnant.Conclusions Prenatal ultrasound diagnosis of fetal aortic atresia has important clinical value.Ascending aorta and aortic arch are thin or unclear,no blood flow connection between fetal aorta and left ventricle and reverse blood flow in aortic arch are major diagnostic evidences.Color Doppler flow imaging is helpful for early diagnosis.

ABSTRACT

ObjectiveTo study the clinical value of prenatal ultrasound in diagnosis of fetal ductus arteriosus stenosis and the guiding significance for clinical delivery.Methods Ultrasound images and perinatal outcome of 12 cases of ductus arteriosus stenosis diagnosed by prenatal ultrasound were retrospectively analysed,the sonographic features were summarized.Results Fetal ductus arteriosus was thin in 12 cases with inner diameter of 1.6 - 2.1 mm.Color Doppler flow imaging showed multicolored blood flow in ductus arteriosus,spectral Doppler showed single peak continuous decreasing high-speed shunt spectrum,peak systolic velocity 256- 404 cm/s,end diastolic velocity 90 - 205 cm/s.Twelve fetuses were associated with right atrial and ventricular dilatation,pulmonary artery widened and moderate to severe tricuspid regurgitation,maximum reflux velocity 324 - 515 cm/s,maximum transvalvular pressure gradient 41 - 106 mmHg.There were 7 cases of emergency cesarean section delivery and 5 cases of oxytocin induced labor delivery,11 cases were full-term infants and 1 case was premature infant.12 fetuses were followed-up after birth by 1 to 3 months of echocardiography until the cardiac structure and function returned to normal,ConclusionsPrenatal ultrasound has important clinical value in diagnosis of fetal ductus arteriosus stenosis,it is helpful to assess the fetal right heart function and guide the clinical delivery.

ABSTRACT

Objective To investigate the clinical value of fetal echocardiography in prenatal diagnosis of complete vascular ring. Methods Ultrasound images of 6 cases with complete vascular ring were retrospectively analysed and summarized, all cases were confirmed by autopsy after termination of pregnancy. Results Right aortic arch and left ductus arteriosus formed U shaped comlete vascular ring coursing around the trachea in 5 cases, left aortic arch and right aortic arch formed O shaped complete vascular ring in 1 case on three-vessel trachea view. Color Dopple flow imaging aided in identifying various components of the vascular ring. Conclusions Fetal echocardiography is of great clinical value in prenatal diagnosis of complete vascular ring. Three-vessel trachea view is important for screening with particular attention to the relationship of the aortic and duct arches to the trachea.

ABSTRACT

Objective To investigate the clinical value of fetal echocardiography in prenatal diagnosis of partial and intermediate atrioventricular septal defect (AVSD). MethodsUltrasound images of11casesof partialandintermediateAVSDconfirmedbyautopsy orpostnatal echocardiography were retrospectively analyzed and the characteristics were summarized. Results Fetal echocardiography of seven cases of partial AVSD and four cases of intermediate AVSD at fourchamber view showed ostium primum atrial septal defect and the mitral and tricuspid valves attaching at the same level on the interventricular septum. Atrio-ventricular valves took on a straight, bar-liked appearance when closed and constituted a “T” shape with interventricular septum. One partial AVSD case showed left ventricle-right atrium canal. All intermediate AVSD cases showed upper ventricular septal defect. There were four cases associated with mitral reflux, one associated with mitral and tricuspid reflux and one with bilateral radius absence. Conclusions “T” shape image is of great clinical value in prenatal diagnosis of fetal partial and intermediate AVSD. The coronary sinus may lead to false positive diagnosis.

ABSTRACT

Objective To investigate the value of prenatal ultrasound observation of subaortic conical connection in the diagnosis of complete transposition of great arteries(TGA).Methods Echocardiography findings of 22 cases with complete TGA confirmed by autopsy were retrospectively analyzed and summarized.Results All cases were shown subaortic conical connection on two-dimensional echocardiography,the two great arteries were parallel in 18 cases on left ventricular long-axis view,the character of normal right ventricular outflow tract surrounding the aortic root disappeared in 17 cases.There were 13 cases with ventricular septal defect and 2 cases with left ventricular outflow tract obstruction.Aorta and pulmonary artery were shown in right anterior and left posterior position among 20 cases,and in anterior and posterior position in 2 cases.Conclusions Prenatal ultrasound observation of subaortic conical connection is of great value in the diagnosis of complete TGA.

ABSTRACT

Objective To investigate the clinical value of fetal echocardiography in prenatal diagnosis of total anomalous pulmonary venous connection(TAPVC). Methods Ultrasound images of ten fetuses with TAPVC were retrospectively analysed and compared with pathologic results. Results Ten fetuses were diagnosed TAPVC by prenatal ultrasound, seven cases were confirmed by autopsy after termination of pregnancy,three ceses were unable to be followed up. Among seven cases confirmed by autopsy, prenatal ultrasound findings showed left atrium were extremely small and no pulmonary vein was observed draining to left atrium. Five fetuses with common pulmonary vein showed widen drainage cavum and high-speed vein spectrum. Three fetuses combined with extracardiac malformations. Conclusions Fetal echocardiography has an important clinical value in diagnosing TAPVC.

ABSTRACT

Objective To explore the value of prenatal ultrasonography for diagnosing fetal congenital diaphragm hernia(CDH). Methods Sonographic imaging of 10 cases with CDH confirmed by autopsy were retrospectively analyzed,the characteristics of imaging were Summarized. Results Nine cases had CDH on the left side,and one case on the right. Nine cases showed occupying lesion in left thoracic cavity with the heart shifting to right,one showed occupying lesion in the right thoracic cavity with the heart shifting to left.Seven cases were detected stomach in left thoracic cavity,one was detected stomach in the ahdominal cavity and 2 cases were not detected stomach in splanchnocoele. All cases were observed that the heart shifted to anterior chest with left atrium separating from descending aorta and the spine. There were 7 cases with hydramnios,3 cases with other organs abnormality among them. Conclusions Prenatal ultrasonography is of great value for diagnosing fetal CDH.

ABSTRACT

Objective To explore the clinical value of ultrosonic diagnosis of fetal Ebstein's anomaly in prenatal.Methods The data of echocardiography and follow-up results of 6 fetal Ebstein's anomaly were retrospective analyzed,and their abnormal ultrasound imaging characteristic were summarized.Results Six cases were proved into Ebstein's anomaly by autopsy,including septal tricuspid leaflet displacement(2 cases),septal and posterior tricuspid leaflets displacement(3cases),septal and posterior and anterior tricuspid leaflets displacement(1 case).There were 4 cases combined with right ventricle dysplasia,1 case with atrial septal defect,1 case with pulmonary stenosis,and 4 cases with hydrothorax,ascibes and pericardial fluid.The echocardiography of fetal Ebstein's anomaly was characterized by right atrial enlargement,displacement of tricuspid valve attachment point,the distance ratio of between cardiac apex and bicuspid valve attachment point and between cardiac apex and tricuspid valve attachment point exceeding(1.8),forming typical atrialized right ventricle feature,color blood flow indicating severe tricuspid regurgitation with low initial point,large area and slow speed(mean speed:(217.0)(cm/s).)Conclusions The fetal Ebstein's anomaly has characteristic sonogram.Ultrasonic diagnosis of fetal Ebstein's anomaly has great clinical value in prenatal.

ABSTRACT

Objective To explore the clinical value of prenatal ultrasonographic diagnosis of fetal pulmonary atresia.Methods The data of echocardiography in 7 fetus with pulmonary atresia confirmed by autopsy were analysed retrospectively,the characteristics of abnormal ultrasound imaging were summarized.Results The dysplastic main pulmonary had been detected only in one fetus by two-dimensional echocardiography,five cases were shown ventricular septal defect with straddle of aortic root on the ventricular septum,two cases with intact ventricular septum had hypoplasis of right heart.Color Doppler flow imaging showed the reverse flow of ductus arteriosus in 5 cases and tricuspid regurgitation in 3 cases.Five cases were diagnosed pulmonary atresia,one case was hypoplasia of right heart,and one case was mistaken for persistent truncus arteriosus.Conclusions Prenatal ultrasonographic diagnosis of tetal pulmonary atresia has great clinical value,the reverse flow of ductus arteriosus is an important clue to diagnosis.
